USPS - if you cannot see the link to create CA-1 and CA-2 forms when you are trying to create these forms please review the following information

USPS has deactivated a claimant’s ability to file a CA-1/CA-2 under the 00 suffix which is the Occupational Health Claims Office. However, with the expansion of the agency code to include the USPS division as the suffix the claimant would need only to select their state and then the dropdown aligned with their division. As an example, a city carrier injured in Minnesota would select Retail and Delivery MN ND when filing a new claim. The drop downs shown below are the selections a claimant can make when filing a new CA-1/CA-2. Even though the ability to submit a CA-1/CA-2 under the 00 suffix is disabled, we did allow for CA-7’s to still be submitted under 00 until we can have all claims reassigned to the correct division.

 

While USPS indicates it has communicated this change, this is not necessarily something of which individual USPS employees have been made.
